2. Please make sure you fill out the Staff Background Information form on the main Staff Paperwork page

Using IdentoGO – has locations outside of Colorado & in Aspen on Wed

• Go here – (find location by selecting a enrollment location)

• Select – Schedule or manage appointment -fill out form to select a time

 Email me a copy of your receipt as we will reimburse you for this expense

Show up for an appointment with required documents. – All results will be sent directly to us.
