Required Medical forms for Campers
Below you will find links to all of the required medical forms we need regarding your camper this summer. All forms can be uploaded electronically using the link found at the bottom of page.
Forms required for ALL CAMPERS
The Parent Form
Bunk requests, pickup locations, general information, and liability form
General Health Appraisal Form
REQUIRES a visitation to a health care provider to complete.
Certificate of Immunization
Need to provide a copy of your camper’s immunization records.
Additional Medical Forms
These forms are REQUIRED for any campers with: allergies, asthma, those bringing medications, or with additional conditions.
Medication Permission Form
Required for ANY medication your camper is bringing with them. One per medication. Any medications brought without this form accompanying it will NOT be accepted.
