Requires a visitation to a health care provider to complete.

Additional Medical Forms

Required for any campers bringing any medications to camp — including over-the-counter meds like Advil, Claritin, and Benadryl. Those with EpiPens and Inhalers must also fill out the care plan below. All must be signed by a medical professional.

Medication Permission Form

One per medication

Required for ANY medication your camper is bringing — including over-the-counter meds like Advil, Claritin, Benadryl, etc. One form per medication. Any medications without this form will NOT be accepted.
